OK - I found the source code for GenericWinDirCtrl.
It has great example code for finding folder, default
exe, and default icons as well as other icons using
the methods people have already pointed out in this
thread.
Looks like finding an icon for 'shortcuts' and for
files that have been reassociated using 'Right click,
open with ..., etc' are still unsolved problems.
Hopefully they will be solved in the future.
